Position Overview

Advanced Cardiology PC is seeking a dedicated and compassionate Full-Time Nurse Practitioner to join our team in Elmira, New York. Work Environment

This is an onsite position located within a school setting, providing a unique opportunity to engage with the community while promoting health education and services. You will join a team of collaborative health professionals committed to enhancing the well-being of our patients in a welcoming and supportive atmosphere. The standard work schedule will be Monday through Friday from 9:00 AM to 5:00 PM, allowing for a work-life balance while serving those in need.

Key Responsibilities

• Provide comprehensive primary care to adult patients, addressing both routine and acute health issues.
• Conduct thorough annual wellness visits and health risk assessments, ensuring accurate documentation and a holistic perspective of each patient's health.
• Coordinate care across various providers, specialists, testing facilities, and community resources to optimize patient outcomes.
• Lead population health initiatives, including routine screenings and preventive care measures in collaboration with the care team.
• Assist in phone triage and outreach efforts to maintain continuity of care and support patient engagement.
• Educate patients regarding their health conditions, care plans, and treatment options, fostering an environment of understanding and partnership.
• Participate in outreach and promotional activities aimed at enhancing community health awareness.
• Conduct home visits when necessary to ensure comprehensive patient care.
• Report to the Medical Director while collaborating closely with operational leadership to enhance clinic operations.
• Perform additional duties as assigned to support organizational goals and patient care quality.

Patient Population / Client Focus

Our primary focus is on older adults enrolled in Medicare, a demographic that requires specialized care strategies to maintain their health and well-being. By building long-term relationships grounded in trust, you will empower patients to take an active role in managing their health, leading to improved outcomes and quality of life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from a recognized body (AANP or ANCC).
• Active, non-probationary Nurse Practitioner license in the state of New York.
• Possession of an active DEA license to prescribe medication.
• Strong foundation in primary care, particularly in internal medicine, geriatrics, or family medicine.
• Proficient in HCC documentation, ICD-10 coding, and execution of Health Risk Assessments (HRAs).
• A genuine passion for interdisciplinary teamwork and delivering patient-centered care.

Preferred Experience

• Previous experience working in a school-based health setting or a similar community-oriented environment.
• Experience with electronic health records (EHR) systems and familiarity with care coordination workflows.
• Demonstrated ability to lead population health initiatives and implement preventive care strategies.

Compensation and Benefits

The salary for this position ranges from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
