Jackson Medical Center, located in the heart of Elmira, New York, is seeking a dedicated and passionate Family Nurse Practitioner (FNP) for a full-time role. Work Environment

The FNP will be required to work onsite at an Assisted Living Facility, offering a full-time schedule typically from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics extending until 8:00 p.m. As part of your role, you may also participate in health fairs and community outreach events on weekends, allowing you to engage directly with the community and enhance public health awareness.

Key Responsibilities

• Perform comprehensive health assessments and screenings, utilizing tools and methodologies to evaluate patient health status thoroughly.
• Diagnose, manage, and treat acute and chronic conditions, developing tailored care plans while integrating preventive care protocols.
• Collaborate with local health organizations and stakeholders to create and implement effective health education programs aimed at improving community wellness.
• Provide culturally competent care by understanding the diverse backgrounds and needs of patients, ensuring they receive necessary health education and advocacy.
• Coordinate and monitor patient referrals to specialist services, ensuring seamless continuity of care and follow-up processes.
• Maintain meticulous and compliant medical records through electronic health record (EHR) systems, ensuring adherence to regulatory guidelines.
• Stay informed about community health trends, emerging medical practices, and relevant public health policies to inform patient care strategies.

Patient Population / Client Focus

Your primary focus will be on individuals residing within the Assisted Living Facility and the wider Elmira community, many of whom may have complex health needs requiring multifaceted care plans. As a Family Nurse Practitioner, you will be integral in fostering relationships with patients, ensuring they feel supported and understood. Qualifications

• A current Nurse Practitioner certification and an active state nursing license are mandatory.

Compensation and Benefits

The position offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
