Pediatric Hospice and Home Health RN

Assignment Duration

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 - 17:00)

Job Description

This position involves providing care in both hospice and home health settings specifically for pediatric patients. The ideal candidate will bring experience in both areas and demonstrate a strong capacity for compassionate patient care.

Responsibilities

• Deliver high-quality nursing care to pediatric patients in hospice and home health environments.
• Work collaboratively with an interdisciplinary team to create and implement individualized care plans.
• Maintain accurate and up-to-date patient records using the HomeCare HomeBase charting system.
• Provide education and support to patients and families regarding care plans and health management.
• Monitor patient health and report changes to the supervising nurse or physician.

Required Experience / Certifications / Licensure

• Prior hospice and home health experience is required.
• Active RN license; both temporary or pending licenses are accepted if confirmation of licensure is provided prior to the start date.
• A minimum of two supervisory references is required.
• Experience with the HomeCare HomeBase charting system is preferred.
