Care Manager (RN)

About This Role The Care Manager (RN) at Centene is responsible for developing, assessing, and facilitating complex care management activities for members with primarily physical health needs. This role is ideal for nurses who are passionate about providing high-quality, cost-effective healthcare outcomes. Highlights Pay: $56,200.00 - $101,000.00 per year Location: Remote, MO Residency: Must reside in MO Schedule: Full Time, Day shift What You'll Do Evaluate the needs of the member and recommend/facilitate plans for the best outcomes. Develop ongoing care plans and collaborate with providers to address unmet needs. Identify problems/barriers to care and provide appropriate interventions. Coordinate between the member, family/caregivers, and care provider team. Provide ongoing follow-up and monitoring of member status and care plan goals. Provide resource support for local services based on assessments. Facilitate care management and collaborate with providers for timely access to care. Perform telephonic, digital, home, and/or other outreach to assess member needs. Collect, document, and maintain member information to ensure compliance. Provide education to members and their families on disease processes and care options.
