Description

• Conduct thorough evaluations of patients in various settings, whether inpatient or outpatient, and create tailored treatment plans that drive results
• Implement effective physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Engage collaboratively with a diverse care team including nurses, physicians, case managers, and rehab professionals to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge plans, and at-home exercises
• Maintain detailed documentation of evaluations, treatments, and patients' progress in alignment with facility and regulatory standards
• Participate actively in team rounds and interdisciplinary meetings to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or a similar accredited degree

Certifications

• Current state licensure as a Physical Therapist
• Completion of CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) valued, but not obligatory

Skills

• Prior relevant experience is highly preferred
