Key Responsibilities\n

• Conduct comprehensive psychosocial assessments and develop individualized treatment plans in collaboration with healthcare team members.

• Provide counseling and crisis intervention sessions to patients, addressing a range of emotional and behavioral issues through evidence-based approaches.

• Maintain accurate and timely clinical documentation, including progress notes and treatment plans, in accordance with departmental policies and regulations.

• Coordinate discharge planning and aftercare services, working diligently to facilitate access to community resources and support systems.

• Consult with families and support networks of clients to ensure a holistic approach to treatment and seamless transition post-discharge.

• Track and report patient progress to physicians and other healthcare professionals, addressing any emerging concerns and adapting treatment plans as necessary.

Patient Population / Client Focus\n

As an LPC at Expression Behavioral Healthcare, you will serve a diverse population of clients in the Erie, Pennsylvania region. Your patient caseload may include individuals facing a variety of mental health challenges, such as anxiety, depression, trauma, and behavioral issues. You will utilize your expertise to provide compassionate care tailored to meet the unique needs of each client, thereby fostering positive health outcomes and personal growth.

Qualifications\n

• A Master’s degree in Professional Counseling, Psychology, or a closely related field from an accredited institution.

• Current licensure as a Licensed Professional Counselor (LPC) or its equivalent.

• A minimum of 3 years of hands-on experience working in an outpatient setting is required.

• Demonstrated ability to deliver evidence-based mental health services effectively.

• Strong organizational skills with the ability to work independently and manage multiple priorities.

• Proficiency in computer applications, including Microsoft Office Suite, for clinical documentation and communication.

• Familiarity with community resources and referral systems in the Erie area.

Compensation and Benefits\n
