Job Summary: Lewis Center, Ohio Physical Therapist Outpatient (PT OP) Outpatient Physical Therapist specializing in pelvic health is responsible for evaluating assigned patients and developing, implementing, and modifying individualized treatment plans based on patient needs, goals, and physician direction. Responsibilities include maintaining timely and accurate medical record documentation and supporting safe, effective progression toward discharge and community activity goals. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; CPR certification; pelvic health specialty; active/applicable license for the State where work will be performed; 1+ year of outpatient clinical experience.
• Preferred Qualifications: None specified.
• Additional Information: Evaluates assigned patients and completes individualized treatment plan development; ensures care is provided under signed, valid physician orders and within insurance approval as indicated and necessary for clinical care; completes medical record documentation including goals and related clinical documentation; participates in assigned student programming with appropriate supervision and learning support; adheres to the discipline-specific State Practice Act and maintains professional licensure in good standing, including completion of state-mandated continuing education; supports performance improvement initiatives and follows through on action plans; demonstrates professional and ethical conduct consistent with role expectations and organizational standards.
