IRIS Care Navigator: Empower Independence (Travel Required)

Lutheran Social Services of Wisconsin and Upper Michigan's GROWING Connection's program seeks an IRIS Consultant to empower participants to live independently. The role supports autonomy, health, and long-term care navigation, traveling across Milwaukee and surrounding counties. You will build relationships, develop IRIS Plans, and maintain regular contact with participants while collaborating with the ICA team to ensure meaningful, client-directed lives. #J-18808-Ljbffr Upper Michigan, Inc.
