Mobile Hospice RN Case Manager | Home Visits
Ennoble Care
Ennoble Care is seeking a Hospice Registered Nurse Case Manager to service patients in Middlesex/Somerset County, NJ. The RN will work with an interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize patient comfort and support families. The role includes caseload management, collaboration with physicians and Hospice staff, and thorough documentation in EMR.
