We are expanding our Maryland team and seeking a Wellness RN who thrives in a supportive, patient‑centered environment. Position Summary The Wellness RN plays a key role in promoting client health, safety, and well‑being through in‑home wellness visits. This role is ideal for a nurse who values autonomy, meaningful one‑on‑one care, and a predictable 4‑day‑per‑week schedule. Responsibilities Conduct in‑home wellness assessments and routine health evaluations Monitor vital signs, medication compliance, and overall client status Provide education to clients and families on health maintenance and safety Document all visits accurately and in compliance with agency standards Collaborate with the care team to support individualized care plans Identify changes in condition and communicate promptly with supervisors Promote independence, dignity, and quality of life for each client Qualifications Active RN license in the state of Maryland Minimum 1 year of nursing experience (home health or community care preferred) Strong assessment and communication skills Ability to work independently and manage a flexible caseload Reliable transportation for home visits Schedule & Benefits Consistent 4‑day‑per‑week schedule Work time 8:30am‑2:30pm Supportive team environment Opportunities for professional growth Competitive hourly pay #J-18808-Ljbffr
