Key Responsibilities\n

As an LCSW at West Valley Psychiatric Services, your responsibilities will include:

• Conducting thorough assessment of clients' counseling needs, focusing on both mental health and substance use concerns.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to diagnose substance abuse issues and determine the appropriate level of care using ASAM (American Society of Addiction Medicine) guidelines.

• Developing and implementing individual treatment plans tailored to each client’s unique needs, firmly grounded in substance abuse counseling.
\n
• Creating and updating discharge plans and continuity of care strategies, making sure clients transition smoothly through different levels of care.
\n
• Documenting all counseling sessions, assessments, and follow-ups meticulously in clients' clinical records to uphold compliance and facilitate care continuity.
\n
• Maintaining an individual caseload, performing case management tasks, and conducting utilization reviews to ensure high-quality service delivery.
\n
• Coordinating essential support services for clients and their families, documenting these in their clinical records to enhance holistic support.
\n
• Engaging with referral sources, such as legal entities, schools, and employers, to establish a robust support system for clients.
\n
• Facilitating interventions during crisis situations, applying a multi-disciplinary team approach to ensure comprehensive care.

Patient Population / Client Focus\n

This role primarily serves clients grappling with substance use disorders within the hospice framework. You will work closely with individuals navigating complex emotional, social, and medical challenges, taking an integrative approach to address their multifaceted needs. This includes addressing co-occurring mental health conditions, an area where you will be expected to apply your expertise and compassion to foster recovery and stability.

Qualifications\n

The successful candidate for this LCSW position will possess:

• A Bachelor's Degree in Social Work, with an emphasis on mental health and substance use counseling.

• A current state license as a Clinical Social Worker (LCSW).
\n
• Demonstrated professionalism, excellent interpersonal skills, cultural competence, self-motivation, and strong organizational abilities.

Preferred Experience\n

While not mandatory, candidates with prior experience in hospice care, substance use treatment, or mental health counseling will be preferred. Familiarity with contemporary counseling methodologies and regulations within the addiction treatment field is highly advantageous.

Compensation and Benefits\n
