RN Medical Care Manager — HIV Care & Resource Navigator

AIDS Healthcare Foundation in Beverly Hills, CA seeks an RN Medical Care Manager to integrate Ryan White Care Act requirements with medical case management. You will conduct Health Risk/Needs Assessments, develop Service/Action/Care Plans, and educate patients about diagnoses and medications, coordinating referrals and resources. As a liaison between providers and patients, you will support vulnerable HIV/AIDS patients, aim for viral suppression, and contribute to HAB measures and program #J-18808-Ljbffr AIDS Healthcare Foundation
