Description

• Conduct thorough evaluations of patients in inpatient or outpatient environments and create tailored treatment plans
• Administer effective physical therapy interventions centering on mobility, strength, balance, and overall function
• Collaborate with an interdisciplinary team of nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes
• Guide patients and their families through therapeutic goals, discharge strategies, and exercises to practice at home
• Keep detailed records of evaluations, treatments, and patient progress, adhering to facility guidelines and regulations
• Engage in care team meetings and interdisciplinary discussions to ensure well-coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution or equivalent qualifications

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications such as neurological or geriatric are advantageous but not mandatory

Skills

• Prior experience is highly desirable
