Description - Conduct patient evaluations in both inpatient and outpatient settings while formulating customized treatment plans. Administer physical therapy interventions that prioritize mobility, strength, balance, and overall function. Collaborate with physicians, nurses, case managers, and rehabilitation professionals to ensure optimal patient outcomes. Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home exercise routines. Document all evaluations, treatments, and progress in compliance with facility protocols and regulatory standards. Engage in care team discussions and interdisciplinary conferences to guarantee coordinated care for all patients.

Education - Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program. Certifications - Valid state licensure as a Physical Therapist. CPR certification is preferred. Additional specialized certifications (such as in neurology or geriatrics) are considered a plus but are not absolutely necessary. Skills - Experience in a physical therapy setting is highly preferred. Proficiency in treating patients with multifaceted medical and mobility challenges.
