Description

• Assess patients in both inpatient and outpatient environments, crafting customized treatment plans
• Deliver physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Instruct patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
• Maintain thorough documentation of evaluations, treatment processes, and patient progress in accordance with established guidelines
• Engage in interdisciplinary rounds and conferences to ensure unified and coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
