Description
- Assess patients in both inpatient and outpatient environments, crafting customized treatment plans
- Deliver physical therapy interventions aimed at improving mobility, strength, balance, and overall function
- Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
- Instruct patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
- Maintain thorough documentation of evaluations, treatment processes, and patient progress in accordance with established guidelines
- Engage in interdisciplinary rounds and conferences to ensure unified and coordinated care
Education
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
- Active state licensure as a Physical Therapist
- CPR certification preferred
- Specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory
Skills
- Prior experience is strongly preferred
- Ability to navigate complex medical and mobility challenges with ease
- Exceptional communication and collaborative skills
- Proficient in utilizing digital charting systems and clinical documentation
