Assess and evaluate patients in inpatient and outpatient environments to create personalized treatment plans

Deliver physical therapy services that enhance mobility, strength, balance, and overall function

Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes

Guide patients and families through therapeutic objectives, discharge processes, and home exercise regimens

Record evaluations, treatments, and patient progress in alignment with facility protocols and regulations

Engage in interdisciplinary care team discussions and rounds to ensure seamless coordination of care

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Active state licensure as a Physical Therapist

CPR certification is preferred

Additional certifications in specialized areas (e.g., neurological, geriatric) are advantageous but not mandatory

Preferred prior experience in similar settings

Capability to treat patients with diverse medical and mobility complexities

Excellent communication and teamwork abilities

Familiarity with digital charting and clinical documentation tools
