Our focus is on creating a supportive space for both caregivers and residents within assisted living communities.Work EnvironmentThis role is primarily onsite, providing therapy directly within assisted living facilities across the Princeton area. Your schedule will typically be Monday to Friday from 9 AM to 5 PM, allowing for a predictable work-life balance. You'll be equipped with mobile electronic medical record (EMR) tools to streamline documentation and ensure HIPAA compliance, enabling you to focus more on delivering high-quality care.Key ResponsibilitiesProvide personalized on-site therapy services to residents in affiliated assisted living communities, ensuring care is tailored to individual needs and conditions.Conduct thorough evaluations to assess residents' physical abilities and develop individualized, orthopedic-informed treatment plans that align with best practices.Collaborate effectively with on-site nursing staff, caregivers, and our multidisciplinary team to coordinate comprehensive care for residents.Educate residents and their families on safe mobility practices, the benefits of home exercise programs, and overall wellness strategies to promote independence.Utilize mobile EMR tools for efficient and accurate documentation of patient progress, treatment plans, and outcomes.Serve as a primary liaison between the assisted living facility and our internal clinic, facilitating referrals and follow-up care support.Patient Population / Client FocusYou will primarily work with older adults in assisted living settings, a demographic that often faces unique physical challenges due to age-related conditions. Your expertise in orthopedic care will be crucial for addressing common issues such as arthritis, post-surgical rehabilitation, and balance disorders. This role allows you to build meaningful relationships with residents while positively impacting their daily activities and mobility.QualificationsCurrent license to practice as a Physical Therapist in the State of New Jersey (or eligibility to obtain licensure).A minimum of 2 years of professional experience as a Physical Therapist, particularly in outpatient, geriatrics, or assisted living facility settings.Strong interpersonal skills, with the ability to communicate effectively with seniors, families, and a multidisciplinary team.In-depth clinical knowledge of orthopedic conditions commonly seen in the aging population.Ability to work autonomously in residential care environments while maintaining collaboration with the facility's staff.Preferred ExperienceExperience in rehabilitation focused on geriatrics, providing tailored therapy solutions.Familiarity with various therapeutic modalities and exercise techniques designed for older adults.Background working within a team-based care model, showcasing an understanding of multidisciplinary collaboration.Compensation and BenefitsSt. Elara Medical Center offers a competitive salary range of $90,000 to $100,000 per year, commensurate with experience.
