Field-Based Community Health Advocate

Molina Healthcare of Illinois

Molina Healthcare seeks a Community Health Worker to support community-based member advocacy and connect vulnerable members with health care resources. You will engage with members in homes, shelters, and provider offices to navigate benefits and health plan options. Responsibilities include coordinating appointments, arranging transportation, and connecting members with social services, while maintaining Medicaid eligibility and following up on missed visits.
