This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.Required QualificationsCurrent Registered Nurse license valid for practice in MissouriCurrent driver's licenseCurrent CPR certificationTwo years of Registered Nurse experience in a clinical care settingOne year of full-time hospice experience preferredReliable transportation and valid automobile liability insuranceAbility to work independently and make sound clinical decisionsStrong written, verbal, organizational, and interpersonal skillsKey ResponsibilitiesCoordinate and oversee all aspects of assigned patients' hospice careDevelop and implement individualized plans of care with the interdisciplinary teamPerform head-to-toe assessments, monitor symptoms, and manage medicationsEducate patients and families regarding condition, needs, and careCommunicate patient progress with clinical leadership, physicians, and the care teamComplete all required documentation accurately and on time
