Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A healthcare facility in State College, PA is seeking a Clinical Pharmacist to join their team in a direct hire capacity, with the possibility of a temp-to-hire arrangement. This role combines clinical and staffing responsibilities, with approximately 60% of your time dedicated to clinical work and 40% to staffing duties.

In this hybrid position, the clinical focus will be centered on Population Health and Ambulatory Care. You will collaborate with patients within the health system to manage medication regimens, particularly targeting chronic disease states such as diabetes. As clinical programs grow, there is strong potential to transition into a fully clinical role.

Schedule:

• Variable days and afternoons, primarily day shifts
• 8-hour shifts

Responsibilities:

• Manage and optimize patient medication regimens in ambulatory and population health settings
• Provide clinical pharmacy services with a focus on chronic disease management
• Participate in staffing duties covering pharmacy operations
• Support the expansion of clinical pharmacy services within the institution

Qualifications:

• Licensed Pharmacist in Pennsylvania
• Experience in ambulatory care or inpatient pharmacy practice preferred
• Completion of residency strongly preferred
• Knowledge of Meditech system is a plus
