Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
An exciting opportunity is available for a Clinical Pharmacist in a hybrid role combining clinical and staffing responsibilities. This unique position allows for meaningful patient interactions while supporting pharmacy operations in a dynamic healthcare setting.
Location: University Park, PA (State College area)Schedule: Variable days and afternoons, primarily day shifts; 8-hour shiftsJob Type: Direct Hire (temp-to-hire considered)
Responsibilities:
- Deliver clinical pharmacy services focused on Population Health and Ambulatory Care, with an emphasis on managing medication regimens for targeted disease states, such as diabetes.
- Provide staffing support approximately 40% of the time to ensure smooth pharmacy operations.
- Collaborate with multidisciplinary teams to optimize patient outcomes within the health system.
- Contribute to the growth and potential expansion of clinical pharmacy programs, transitioning towards a fully clinical role as opportunities develop.
Desired Experience and Qualifications:
- Licensed Pharmacist in Pennsylvania.
- Experience in ambulatory care or inpatient pharmacy practice.
- Post-graduate residency training strongly preferred.
- Strong patient care orientation and proactive approach to clinical program development.
- Familiarity with Meditech electronic health record systems is an advantage.
