A healthcare system in State College, PA is seeking a dedicated Clinical Pharmacist to join their team in a hybrid clinical and staffing role. This position offers the unique opportunity to provide comprehensive medication management with a focus on Population Health and Ambulatory Care, particularly targeting chronic disease states such as Diabetes.

In this role, you will spend approximately 60% of your time delivering direct clinical care to patients within the health system, optimizing medication therapies and improving outcomes. The remaining 40% involves traditional staffing responsibilities, ensuring efficient pharmacy operations. There is potential for the role to evolve into a fully clinical position as the program expands, making this an excellent opportunity for a pharmacist passionate about patient care and program development.

Key Responsibilities:

• Manage medication regimens for patients with chronic conditions, focusing on population health initiatives.
• Collaborate with interdisciplinary teams to develop and implement patient-centered clinical programs.
• Provide staffing support including order verification and medication dispensing as needed.
• Participate in quality improvement and clinical service expansion efforts.

Desired Experience and Qualifications:

• Licensed Pharmacist in Pennsylvania.
• Experience in ambulatory care or inpatient pharmacy practice preferred.
• Completion of a residency program is highly desirable.
• Familiarity with Meditech electronic health record system is a plus.
• Strong communication and patient care skills.

Schedule:

• Variable days and afternoon shifts, mainly 8-hour day shifts.

This is a direct hire opportunity with a competitive salary range from $106,000 to $165,000, reflecting experience and qualifications.
