Registered Nurse - Case Manager

Role Overview Provides discharge planning for patients by assessing needs, developing care transition plans, and collaborating with interdisciplinary teams. Ensures compliance with discharge regulations, identifies barriers to timely discharge, and addresses high‑risk readmissions through root cause analysis and care coordination. May handle utilization management, emergency department case management, admission processes, and liaise with post‑acute providers. Applies evidence‑based practices and adheres to professional nursing standards, including the ANA Code of Ethics, ensuring patient‑centered care tailored to age and clinical needs. Qualifications Graduate of an accredited nursing program with at least three years of acute care experience. Must hold an active RN license valid in the applicable state(s). Physical ability to perform frequent lifting, movement, and data entry tasks.
