Registered Nurse - Case Manager

About the Role Case Management at Mary's Hospital – a demanding, compassionate role where you help patients transition from hospital to the next level of care. Schedule & Compensation • Work Hours: MondaythroughFriday, 8:00AM – 4:30PM • Regular full‑time position (40hours per week), Day Shift Job Summary Assesses, develops, plans, implements, and evaluates patients’ discharge planning needs in collaboration with an interdisciplinary team, ensuring a smooth transition to appropriate post‑acute care. Key Responsibilities Ensure compliance with Condition of Participation (CoP) discharge planning standards. Identify and elevate gaps or barriers related to inpatient and observation length of stay. Assess patients at high risk for readmission and identify root causes. Develop and collaborate on discharge plans with a multidisciplinary team. Participate in continuous improvement initiatives for standard work and quality. May manage utilization, ED case management, admission processes, or serve as liaison with post‑acute providers such as skilled nursing facilities, rehab, and home health. Apply evidence‑based practice and the ANA Code of Ethics to guide nursing care tailored to patient age and clinical needs. Perform other duties as assigned. Education & Experience Graduate of an accredited school of nursing or education equivalency for licensing. Minimum 3years acute‑care experience. Physical Requirements Frequent lifting/carrying and pushing/pulling 0‑25lb objects; occasional lifting 25‑50lb. Frequent seating, standing, walking, reaching, and repetitive footorhand/arm movements. Frequent use of vision and depth perception (20inches or less and 20feet or more); frequent use of hearing and speech for communication. Frequent keyboard use and data entry; occasional bending, stooping, kneeling, squatting, twisting, and gripping. Rare climbing. License & Certifications Registered Nurse (RN) issued by the Missouri Division of Professional Registration or a Compact State.
