In this full-time remote position, you will provide critical counseling services directly from a Hospice Center in Woodbridge, New Jersey. Your role will be pivotal in supporting individuals and families impacted by substance use disorders and mental health challenges, ensuring that each client receives the tailored treatment they need for successful recovery.

This LCSW position is designed to support remote work, with the opportunity to engage with clients virtually while being part of a multidisciplinary team. Key Responsibilities\n

  Conduct comprehensive assessments to identify the counseling needs of clients, utilizing the DSM (Diagnostic and Statistical Manual) criteria to determine appropriate substance abuse diagnoses and levels of care.

  Develop and implement individualized treatment plans based on clinical assessments, ensuring a focus on evidence-based strategies and holistic recovery approaches.

  Document all aspects of the counseling process, including services rendered, assessments, recommendations, and ongoing evaluations in each client’s clinical record.

  Maintain an active caseload, conducting regular treatment planning discussions and adjusting care strategies as needed to align with client progress.

  Coordinate essential support services for clients and their families, ensuring a comprehensive approach to recovery.

\n
• \n

  Facilitate interventions for crisis-oriented issues, collaborating with a multidisciplinary team to address immediate needs and long-term recovery goals.

  Establish effective communication with referral sources such as legal entities, educational institutions, and employers to support client progress and continuity of care.

Patient Population / Client Focus\n

In this role, you will primarily work with clients experiencing co-occurring disorders that involve substance use and mental health challenges. Your expertise will be crucial in providing stabilizing support, leading counseling sessions, and promoting recovery-focused strategies that significantly improve patient outcomes.

Qualifications\n

  A Bachelor’s degree in social work is required; however, a Master’s degree and LCSW licensing is necessary for the position.

  Demonstrated professionalism, cultural competency, and a strong understanding of therapeutic boundaries.

  Self-motivated and organized with excellent time management skills conducive to a remote work environment.

  Proficient in utilizing electronic health record (EHR) systems for documentation and case management.

Preferred Experience\n

  Experience in substance use disorder treatment and familiarity with ASAM (American Society of Addiction Medicine) criteria.

  Background working in hospice or palliative care settings is an advantage.
