Encompass Rehabilitation Center is currently seeking a dedicated and compassionate Physical Therapist to join our team in Culpeper, Virginia. This is a full-time, onsite position based in the home care setting, where you will provide exceptional rehabilitation services to patients in the comfort of their own homes. Work Environment

In this role, you will work directly within clients' homes, facilitating a comfortable and supportive environment for rehabilitation. You will join a multidisciplinary team composed of healthcare professionals who collaborate closely to implement and analyze individualized treatment plans, ensuring holistic care tailored to every patient.

Key Responsibilities

• Develop and implement personalized physical therapy treatment plans aimed at preventing disabilities and relieving pain, while improving and restoring muscle function.
• Supervise Physical Therapy Assistants as necessary, ensuring they are executing the prescribed plan of care effectively.
• Conduct thorough evaluations of clients' physical abilities through diagnostic tests, including assessments of muscle, nerve, joint, and functional abilities.
• Assess clients' home environments to identify modifications that can enhance their independence in daily living.
• Perform a variety of therapeutic interventions, including manual therapy, therapeutic exercises, and the application of physical modalities such as traction and massage.
• Evaluate, fit, and make necessary adjustments to orthotic and prosthetic devices, providing recommendations for modifications based on patient needs.
• Document patient progress meticulously, preparing clinical records that align with company standards, including initial evaluations, progress notes, and care plans.
• Participate actively in interdisciplinary team meetings to discuss client care, treatment outcomes, and adjustments to care plans as needed.
• Instruct clients and their families on the proper use of physical therapy equipment and overall management of their therapy programs.
• Ensure compliance with all applicable regulations and standards in documentation and clinical practice.

Your expertise will directly impact a diverse range of clientele, including individuals recovering from surgeries, stroke patients, older adults managing chronic conditions, and those needing post-acute care. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the state of Virginia, in good standing.
• Current CPR certification and a valid driver's license.

• Strong communication skills to effectively engage with patients, families, and interdisciplinary team members.
• The ability to work collaboratively within a team and demonstrate leadership qualities.

Preferred Experience

While a minimum of two years of experience is required, candidates with extensive experience in home health care, acute care settings, or rehabilitation will be given preference.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 110,000 annually, complemented by a comprehensive benefits package.
