Job Details Job Location: Haven for Hope of Bexar County - San Antonio, TX 78207 Position Type: Full Time Salary Range: $44,000.00 - $45,000.00 Travel Percentage: Non-driving Role Job Shift: 8:00 AM - 5:00 PM (Monday - Friday) SUMMARY The Intake Specialist will work Monday through Friday. The Intake Specialist will triage, assess and provide placement for clients who enter Haven for Hope’s Intake Department. ESSENTIAL DUTIES AND RESPONSIBILITIES Provide excellent and thorough customer service to all individuals who enter Intake Department. Effectively engage homeless individuals and families, to retrieve the information necessary to enroll in the information tracking system and to assess for appropriate placement or referral. Performs a variety of clerical tasks related to guest/member intake ensuring proper documentation of a guest or member. Requires entering demographic data, attention to detail and accuracy in relaying and documenting personal information, and responding effectively by telephone, and e-mail. Act as a liaison between the member, case managers, and referring agencies while protecting and maintaining member confidentiality. Utilize and develop ongoing clinical skills for assessment of clients, including but not limited to motivational interviewing, systems theory and strengths based perspective. Provide timely and accurate concurrent documentation of assessment and placement, using the Homeless Management Information System (HMIS). Develop thorough knowledge of community area resources for referrals, including but not limited to financial, mental health, substance abuse, and alternative housing resources. Communicate and collaborate with the case management team and Haven for Hope partners in an effort to facilitate successful service delivery. Attend and participate in team meetings, trainings, and special events as needed. Educates, and ensures partners and other referring agencies are following the correct referral policies and protocol for service delivery. Support the goals of the Haven for Hope Campus, promoting a positive Campus image to the local community and adhering to the agency mission as well as demonstration of following a professional code of ethics. Model responsibility and accountability while supporting Haven for Hope mission and goals. Phone coverage which includes answering and directing phone calls. Other duties as assigned. QUALIFICATIONS Minimum High School Diploma or GED. Preferred Bachelor’s degree in a Human/Social Services, Psychology, Social Work, or related field. EXPERIENCE Three years of direct case management or outreach and engagement. Experience working with chronic homeless is preferred. Knowledge and application of case management skills essential with the ability to balance and prioritize simultaneous tasks in a fast-paced and changing environment. KNOWLEDGE/SKILLS/ABILITIES Technical Skills: Proficient ability to navigate in Microsoft Office. Includes ability to learn and maintain information systems developed specifically for Haven for Hope. Ability to learn and apply concurrent documentation techniques is essential. Communications: Proficient in verbal/written communication skills, in both formal and informal settings. Excellent ability to organize information, manage multiple tasks and use available tools to effectively contribute to a team environment. Ability to express oneself professionally within groups and in one-on-one conversation with staff, Prospects and Partners of the Campus. English and Spanish speaking is preferred. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Experience working with the homeless is highly desirable. Excellent time management skills and the ability to effectively communicate both orally and written is essential. The individual must have a high level of energy, creativity and flexibility. This position requires strong interpersonal, clinical, and organizational skills along with a warm, friendly and consumer-centered approach. Candidates for this position will need to be able to form strong working relationships with people sleeping outside in order to facilitate placement at Haven for Hope, other community emergency shelters, assist with the navigation of various social service programs, complete related data entry. CERTIFICATES, LICENSES, REGISTRATIONS Prefer a valid Texas Driver’s License, a good driving record, reliable transportation and proper insurance coverage on automobile used for work. Monthly reimbursement for applicable and documented work related expenditures is provided. Travel within the city and its surrounding areas will be required (if applicable).
