About the Position

Job Title: RN Profession: Nursing Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 12 hours Experience: Two (2) years of experience in a home setting within the last ten (10) years. License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: Experience conducting comprehensive assessments of patients. Experience working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Conducting assessments similar to UAS or OASIS. Providing primary care to homebound individuals. Care management for individuals receiving home and community-based services. Home care experience as a paraprofessional or Licensed Practical Nurse caring for multiple patients per week. Description: RNs will provide telehealth until ready for face-to-face visits. RNs need to be flexible to transition between telehealth and in-field visits based on needs. Nurses must have recent experience conducting comprehensive assessments of patients. Essential skills include the ability to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates should be comfortable working with technology in the workplace. Knowledge of nursing principles and state regulations governing nursing practices is required. Excellent organizational, interpersonal, written, and verbal communication skills are essential. Candidates should be able to communicate effectively with diverse consumers. Sensitivity to the concerns of others is a must. Ability to perform comfortably in a fast-paced, deadline-oriented work environment is necessary. Candidates must successfully execute multiple complex tasks simultaneously and work independently. Responsibilities include performing initial in-home evaluations of patients, reviewing medical documentation, and entering evaluation data into an electronic format. Nurses must document concerns and coordinate with consumers on health care options. Communication with interdisciplinary care team members is crucial for improving care coordination. Collecting data for quality review and maintaining knowledge of community resources are part of the job. Candidates must meet all standards established for this position and perform other duties as assigned.
