About the Position

Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 12 Experience: Two years in a home setting within the last ten years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: - Two years of experience in a home setting. - Recent experience conducting comprehensive assessments of patients. Description: RNs will provide telehealth services until they are ready for in-field visits. RNs must be flexible to transition between telehealth and in-field visits based on needs. Candidates must have experience working in a field for a Certified Home Health Agency, a Licensed Home Care Service Agency, or Managed Long-Term Care. Experience should include conducting assessments similar to the UAS, such as OASIS. Providing primary care to homebound individuals is required. Care management experience for individuals receiving home and community-based services is essential. Prior home care experience as a paraprofessional, or as a Licensed Practical Nurse caring for a minimum of two patients per week is needed. Experience with UAS-NY assessments, especially in the past 365 days, is preferred. Experience caring for a geriatric population and managing their chronic diseases, especially in a home setting, is preferred. Combination of home care experience as a paraprofessional or LPN and RN experiences is highly regarded. Candidates must understand they will be conducting hybrid tele and in-field assessments. All candidates must have the ability to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates should be proficient in technology relevant to nursing. Knowledge of nursing principles and laws governing practices is critical. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Candidates must be able to communicate effectively and sensitively with a multi-generational consumer group. Ability to work in a fast-paced, deadline-oriented work environment is essential. Candidates should demonstrate capability to execute multiple complex tasks simultaneously and work independently. Responsibilities include performing initial in-home evaluations of patients, reviewing medical documentation, and entering evaluation data into electronic forms. Candidates will document concerns and issues that arise during evaluations. Enhancing communication and collaboration with interdisciplinary care team members is vital to improve care coordination. It is necessary to maintain knowledge of community resources, payer requirements, and network services for the target population. Additionally, candidates will collect quality review data and identify opportunities for health promotion and illness prevention.
