Key Responsibilities\n

As a Licensed Master Social Worker (LMSW) in our hospice center, you will hold a variety of responsibilities critical to patient welfare. Your day-to-day tasks will include:

• Conducting thorough clinical assessments to develop individualized treatment plans aligned with best practices and patient needs.

• Providing evidence-based psychotherapy, employing modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to each patient’s circumstances.

• Maintaining comprehensive clinical documentation that complies with HIPAA regulations to ensure confidentiality and high standards of care.

• Collaborating closely with psychiatrists, psychologists, and other healthcare professionals to provide integrated care and address complex patient needs.

• Engaging in regular case consultations and peer reviews aimed at enhancing treatment approaches and overall service quality.

• Equipping patients and their family members with vital information concerning mental health, treatment pathways, and available community resources.

• Staying updated on mental health research trends to inform therapeutic practices and improve patient outcomes.

• Managing a full caseload while ensuring timely progression towards treatment goals and adherence to ethical guidelines as set forth by the National Association of Social Workers (NASW).

Patient Population / Client Focus\n

Your work will largely focus on individuals facing end-of-life issues and their families, addressing a range of emotional challenges such as grief, loss, anxiety, and depression. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Master Social Worker (LMSW) license in New York State.

Compensation and Benefits\n
