Key Responsibilities\n

• Conduct in-depth assessments that evaluate the social, emotional, and spiritual needs of patients and their families. Craft personalized treatment plans based on these assessments, integrating evidence-based practices and solution-focused interventions.

• Implement and oversee therapeutic interventions that cater to individual patient circumstances, such as cognitive-behavioral therapy (CBT) techniques tailored for those experiencing grief and loss.

• Maintain meticulous clinical documentation in accordance with HIPAA guidelines, ensuring the confidentiality and security of sensitive patient information. Timely record-keeping will uphold organizational standards and enhance patient care quality.

• Engage in collaborative discussions with a diverse treatment team, including physicians, nurses, and chaplains, to provide comprehensive care and support to both patients and their families.

• Lead and participate in regular case reviews to discuss patient progress and adapt care plans as needed, drawing on peer insights and up-to-date research in the field.

• Offer education to patients and families about what to expect during hospice care, including mental health support resources available in the community, fostering informed decision-making and resilience.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience.
