Key Responsibilities\n

• Design, implement, and manage individualized support packages tailored to address clients’ unique challenges, whether they face mental health issues, grief, or chronic illness.

• Conduct comprehensive intake assessments by interviewing clients to evaluate their current emotional state, needs, strengths, and vulnerabilities.

• Create personalized and measurable treatment goals that cater to various aspects of the clients’ lives, including mental health, social well-being, and practical living skills.

• Continuously monitor client progress and adapt treatment plans in response to their evolving needs, using evidence-based practices to inform adjustments in care.

• Provide information and counseling about coping strategies and community resources during sessions, enabling clients to make informed decisions about their care.

• Maintain meticulous and confidential records of client interactions, treatment progress, and case evaluations, ensuring compliance with clinical guidelines and privacy regulations.

• Participate in case conferences with multidisciplinary teams, collaborating to develop cohesive plans for care, and may serve as a key-worker in client cases.

• Generate referrals to relevant community services that can assist clients in their recovery journeys and facilitate access to essential support.

Qualifications\n

• A bachelor's degree in Social Work (BSW), Psychology, Sociology, or a closely related field is required; a Master’s degree in Social Work (MSW) is preferred.
