Key Responsibilities\n

As a Licensed Master Social Worker at The Enlightened Mind Counseling Center, your day-to-day responsibilities will include:

• Conducting comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, mental, and social factors influencing their well-being.

• Developing tailored care plans in partnership with clients and family members, focusing on their specific goals, preferences, and challenges.

• Providing individual and family counseling to help seniors navigate transitions such as end-of-life considerations, loss, or changes in health status.

• Advocating for clients' rights to ensure their access to necessary community resources and services, enhancing their overall support network.

• Collaborating with a multidisciplinary team to address complex care needs and improve client outcomes through coordinated interventions.

• Conducting regular follow-up visits to monitor clients’ conditions, assess their living environments, and address safety or health concerns.

• Facilitating communication within families to resolve conflicts and encourage a supportive caregiving environment.

• Educationally empowering clients and their families about available resources, support groups, and programs to foster engagement and enhance their quality of life.

• Maintaining thorough documentation of client care plans, progress notes, and relevant assessments while adhering to ethical standards and practices.

The primary focus of this role is on the senior population within our hospice center. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Master Social Worker (LMSW).
