Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths and challenges associated with their diagnoses.

• Formulate, develop, and implement individualized treatment plans utilizing evidence-based social work interventions tailored to the unique needs of each patient.

• Collaborate closely with a multidisciplinary healthcare team, including physicians, nursing staff, and other specialists, to create a cohesive patient plan that addresses medical, emotional, and practical support needs.

• Facilitate discharge planning that ensures appropriate follow-up care, including necessary resources, referrals, and support systems to promote successful transition after hospitalization.

• Provide education and counseling to patients and families about coping strategies, adjustment to illness, and available community resources.

• Employ a variety of therapeutic modalities, such as crisis intervention, individual, group, and family therapy to address complex patient needs effectively.

• Act as a resource for healthcare colleagues by sharing knowledge and best practices in psychosocial oncology.

• Evaluate oncology patients specifically for mental health concerns such as depression and anxiety while offering short-term therapeutic support.

• Participate in an on-call rotation to address after-hour inquiries and urgent patient needs.

• Present at educational conferences, representing the center as an expert in psychosocial oncology practice.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience.
