Life Stage Counseling is seeking a dedicated Licensed Master Social Worker (LMSW) to join our team, focusing on providing tailored support to the senior population in Trenton, New Jersey. This full-time role will allow you to utilize your skills and expertise to enhance the quality of life for seniors, working primarily in a remote capacity for a detox center. You will engage directly with clients, navigating their psychosocial needs and ensuring they receive the comprehensive care necessary for their well-being.

Work Environment\n

You will operate in a remote work setting, primarily communicating with clients via telehealth platforms and phone consultations. While many tasks can be conducted online, some in-person visits may be required at the detox center to check on clients’ well-being, assess their living conditions, and ensure safety. Working hours are generally scheduled from Monday to Friday, 8:00 AM to 4:30 PM, with the potential for flexibility based on client needs, including occasional evening or weekend hours.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify each senior client's emotional, mental, and social needs.

• Collaborate with seniors and their families in formulating personalized care plans tailored to specific objectives, desires, and family dynamics.

• Provide emotional and counseling support, facilitating dialogue and aiding clients in navigating transitions such as medical changes, loss, or other life challenges.

• Advocate vigorously for clients' rights, empowering them to access community resources and services essential for their health and welfare.
\n
• Work closely with interdisciplinary teams including medical professionals, caregivers, and family members to coordinate ongoing care effectively.

• Perform follow-up visits or calls to assess clients’ progress and make necessary adjustments to their care plans.
\n
• Educate clients and their families about available support mechanisms, community services, and engagement opportunities that enhance social interaction.

• Document client assessments, care plans, and other interactions in detail within our electronic health record (EHR) system to ensure compliance and continuity of care.

• Stay updated on relevant social work practices, policies, ethics, and community resources to provide the highest level of care.

Patient Population / Client Focus\n

This role places you at the heart of a community-centered approach, primarily focusing on geriatric clients who face various challenges related to aging, mental health, substance use, and social isolation. Your work will significantly impact clients transitioning into or out of treatment at our detox center, ensuring they receive uninterrupted support throughout their recovery process and ongoing life adjustments.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• Active and valid state licensure as a Licensed Master Social Worker (LMSW).

• Minimum of two years of professional experience in social work, particularly in geriatric settings or community service environments.

• In-depth knowledge of social work principles, aging issues, and resources within the community.

• Strong assessment, counseling, and communication skills, enabling effective rapport with clients and families.

• Ability to work collaboratively with health care teams and manage client relationships sensitively and effectively.

• Proficiency in electronic health record (EHR) systems and various computer applications.
