Key Responsibilities\n

• Conduct initial assessments and define treatment plans tailored to individual client needs, focusing on evidence-based practices.

• Engage in crisis intervention techniques and provide immediate support for clients in distress.

• Facilitate individual, family, and group therapy sessions, ensuring that therapeutic strategies align with the clients' expressed goals and presenting issues.

• Collaborate with a multidisciplinary team to coordinate care, share insights, and deepen the collective understanding of each client's background and treatment progress.

• Create and implement integrated care plans, ensuring seamless access to necessary services and ongoing communication amongst clients, family members, and providers.

• Utilize electronic health record systems to maintain accurate client documentation, track progress, and ensure timely reporting.

• Monitor and assess client situations throughout their treatment journey, adjusting care plans as needed based on observed outcomes.

• Participate in routine agency meetings and committees aimed at optimizing service delivery and exploring quality improvement strategies.

Qualifications\n

• Master’s degree in social work from an accredited program, along with a current LCSW licensure in New Jersey.

Compensation and Benefits\n
