Sign-on Bonus: $3,500 for FULL-TIME candidates only POSITION TITLE The RN Subacute job description will provide the scope of the position in the facility. Occupational Exposure: Category I Department: Nursing Reports to: Director of Nurses FLSA: Non-Exempt POSITION SUMMARY: Responsible and accountable for the provision of direct, age specific, resident care to those assigned to his/her care for each established shift. The RN provides resident care according to the nursing processes of the facility and is responsible for the coordination of the team approach to meet each resident's individual needs. The RN provides clinical leadership for the other nursing staff as assigned and performs related duties as required. The RN supervises the LVN's, C.N.A.'s, and other ancillary nursing personnel involved in the delivery of resident care. POSITION RELATIONSHIP: Collaborates with all levels of unit nursing personnel and administrative and financial staff, physicians, consultants to the facility. QUALIFICATIONS/REQUIREMENTS: Licensure/Certification: a. Current, valid, Registered Nurse licensure to practice in the State of California b. Current BCLS certification Education: a. RN graduate from an accredited School of Nursing with a minimum of an Associate's Degree Experience: a. Requires at least six months prior RN experience in the past two years in a general acute care hospital setting, or acquired equivalent competency appropriate to the type of sub-acute residents that the facility provides care to. Language Skills: a. Must be able to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language. Mathematical Skills: a. Must be able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. b. Ability to perform these operations using units of weight measurement, and volume. c. Ability to perform upper level mathematical operations. POSITION RESPONSIBILITIES/DUTIES: Performs assigned resident care duties in a manner that provides for the physical, psycho-social, and spiritual needs of the chronically ill and technologically dependent resident, incorporating cultural and ethnic factors. Completes initial and ongoing assessments in a timely manner, incorporating functional/development age factors into the assessment process. Correctly differentiates between normal and abnormal clinical findings and intervenes in accordance with clinical standards of practice and per physician orders. Works collaboratively with the resident/family and interdisciplinary team members to develop an individualized plan of care for each resident. Demonstrates sound clinical judgment in the implementation, and evaluation of the nursing aspects of interdisciplinary resident care plan of care. Proficiently and accurately evaluates, assists with treatments, and reports resident condition changes to the attending physician, family, interdisciplinary team members, and Clinical Manager. Directs and coordinates the care of each resident to meet their overall individualized needs. Administers medications in a proficient manner, including IV therapy and pain management. Is able to accurately calculate dosage and solutions appropriate to resident's age, as needed, including IV therapy, IV pain management, and TPN/PPN. Is able to provide care and treatment to residents with Central Lines, PICC lines, and peripheral IV's. Provides treatment administration in a proficient manner per direction from the physician, which includes but is not limited to: wound packing, irrigation, indwelling catheter care, etc Proficiently provides and manages the care of technologically dependent residents, including but not limited to: a. Ostomies such as, but not limited to: tracheostomy, ileostomy, urostomy, gastrostomy etc. b. Enteral feeding tubes c. Suctioning Proficiently provides care in emergency situations and follows established emergency procedures and practices. Maintains knowledge of and demonstrates the ability to identify therapeutic resident care issues/problems and determines measures and interventions to promote, maintain, and/or restore health of residents. Assures that the rights of residents are respected and maintained by allowing for privacy and dignity in the provision of care. Identifies resident/family educational needs; initiates and provides individualized teaching which considers the functional/developmental age and needs of the resident. Evaluates and documents the effectiveness of the education provided. Demonstrates knowledge of age specific care, including but not limited to identifying safety measures, physiological normal values/reading; and assessing skin integrity, behavior, motor skills and/or activities that place the residents at risk as well as communicates effectively in a clear, concise, and understandable manner. Demonstrates knowledge and appropriate assessment of signs/symptoms of abuse and/or neglect of residents. Reports and documents suspected abuse/neglect in accordance with facility policy and regulatory guidelines. Demonstrates good oral and written communication skills with residents, families, physicians, additional healthcare providers, and co-workers. Timely, accurately, and thoroughly prepares documentation in a manner that conforms to prescribed style and format, and to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. #J-18808-Ljbffr
