This role offers a remote work setting, enabling you to effectively engage with families and address their needs in the comfort of their own environments. As part of our Community Health Clinic, you'll work collaboratively with a team of healthcare professionals to enhance the wellbeing of children and their families, utilizing evidence-based practices to ensure positive health outcomes.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine the unique needs of children and families.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP) to support tailored interventions.
\n
• Collaborate with colleagues in monitoring and assessing outcomes to contribute effectively to team-based care.
\n
• Facilitate family support sessions that empower parents and caregivers with the skills and knowledge necessary for enhancing their child’s development.
\n
• Ensure compliance with all relevant Federal and State regulations.
\n
• Establish rapport with children and families to foster a supportive and nurturing learning environment.

• Utilize coaching models and natural teaching methods to help families engage in their child’s early learning experiences.
\n
• Provide ongoing recommendations for service frequency based on developmental assessments and familial priorities.
\n
• Maintain high standards of health and safety for all employees and clients.

Patient Population / Client Focus\n

This position centers around children who may be experiencing developmental delays or disabilities during their critical early years. The LCSW will work closely with infants and toddlers and their families, employing strategies that promote physical, cognitive, social, emotional, and communication skills essential for their growth.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work is required.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) from the Department of Public Health.
\n
• A valid driver's license is necessary for home visits when required.
\n
• Demonstrated ability to adapt to various learning styles tailored to each child's individual treatment plans.

• One or more years of experience working directly with children in early intervention settings.
\n
• A solid commitment to implementing Natural Learning Environment Practices.

Compensation and Benefits\n
