Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The role focuses on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and departmental policies and procedures. Regulatory standards and professional guidelines also inform nursing practice. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and across other disciplines. Key responsibilities include assessment of patient needs and performing comprehensive assessments in accordance with the RN's scope of practice. The RN considers the patient's culture, age, and developmental level. Identifying patient risk factors and obtaining health information from family or others as necessary is crucial. The RN collects both objective and subjective data and verifies initial admission assessments, if obtained by other members of the healthcare team. Developing a prioritized problem list based on input from the inter-professional team is essential. Actions and interventions are designed to be relevant to the patient’s culture and age. Determining desired outcomes in coordination with the patient and family is performed in accordance with the medical plan of care. The RN fully implements patient care therapies and procedures with clinical autonomy within established policies and procedures. Explaining procedures, treatments, and processes to the patient and family is a key aspect of the role. The RN initiates actions to alleviate patient stress and discomfort and recognizes, responds to, and communicates changes in patient condition appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented according to established standards. Delegation of tasks is managed appropriately while overseeing team members in their patient care activities. The RN monitors the accuracy and completeness of delegated patient care provided by the team. Identifying and addressing educational needs of patients related to prioritized problems is necessary. Facilitating the patient discharge process by executing discharge orders on time and coordinating the care plan is part of the role. Providing input into patient/nurse assignments is considered based on staffing and patient acuity. Demonstrating active participation in unit-based activities in support of organizational values is important. Promoting a positive group dynamic and healing environment is essential through words and actions. Assuring nursing excellence in accordance with accepted standards and evidence-based practice is a priority. Active participation in department and/or unit quality improvement activities is expected. A holistic perspective regarding patient care is utilized.
