Position Overview

Trails Rehabilitation & Wellness Pro is actively seeking a dedicated and experienced Licensed Physical Therapist (PT) to join our dynamic team located in Chantilly, Virginia. This full-time role primarily focuses on providing exceptional physical therapy services in a home care setting. The expected salary for this position ranges from USD 100,000 to 120,000 annually, reflecting our commitment to compensating our healthcare professionals competitively based on their expertise and experience.

Work Environment

This position will require on-site engagement in the homes of our patients, allowing for personalized and direct care. The successful candidate will work Monday to Friday, from 8:00 am to 5:00 pm, contributing to a predictable schedule that allows for a balanced work-life dynamic. Our home care setting provides a unique opportunity to establish close relationships with patients and their families, enhancing their rehabilitation journey.

Key Responsibilities

• Review and understand the state practice acts relevant to physical therapy to ensure compliance with regulations governing practice and supervision standards.

• Conduct comprehensive examinations and evaluations of patients, identifying functional limitations and disabilities to create tailored treatment plans that align with their individual needs.

• Implement and document treatment protocols based on assessments, making adjustments as necessary to optimize patient outcomes and respond to evolving health conditions.

• Consult proactively with interdisciplinary healthcare teams to discuss patient care strategies and adapt treatment plans as changes in patient conditions necessitate.

• Provide thorough documentation of patient evaluations, treatment plans, progress notes, and discharge planning, ensuring clarity and compliance with current healthcare documentation standards.

• Engage in ongoing professional development by attending team meetings and training sessions, and participate actively in efforts to remove barriers to accessibility in patient care.

• Assess, treat, and provide direction for patients requiring physical therapy, ensuring medical necessity for therapy services is well documented.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population dealing with various orthopedic conditions. Utilizing evidence-based practice, the PT will not only focus on treating physical impairments but also foster connections with patients and families to enhance engagement in their rehabilitation process. This role is critical in facilitating improvements in mobility, strength, and overall quality of life for our patients.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• Current state licensure as a Physical Therapist in Virginia, as sanctioned by the Physical Therapy Examining Committee.

• Two years of outpatient orthopedic experience, ideally in a home care setting, demonstrating a strong foundation in orthopedic rehabilitation.

• Current CPR certification, ensuring readiness to respond to emergencies in the home care environment.

Preferred Experience

While a minimum of two years in outpatient orthopedic care is required, candidates with experience in home health settings or with specialized certifications in orthopedic therapy will be highly favored. Familiarity with telehealth practices and technologies will also be considered an asset, as they can complement in-person therapy initiatives.
