Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the needs of patients and tailor treatment plans accordingly.

• Facilitate communication among supervisors, family members, and various stakeholders regarding patient progress and treatment strategies.

• Create and manage a schedule of virtual appointments, ensuring timely follow-ups and progress evaluations.

• Coordinate and execute home visits as needed to gather insights from clients in their natural environments, when appropriate.

• Implement and monitor program plans to assure they meet the therapeutic goals set forth in collaboration with the patient.

• Actively participate in interdisciplinary team meetings, contributing to strategic discussions about patient care.

• Document monthly and quarterly progress notes to maintain accurate patient records and inform treatment planning.
\n
• Lead training sessions for team members on the effective implementation of therapeutic programs and documentation requirements.

• Provide ongoing support through follow-up interviews and virtual visits, adapting care as clients progress.

• Serve as a liaison between clients and the wider mental health community to promote awareness and access to resources.

• Contribute to the development of training materials aimed at enhancing the effectiveness of mental health programs.

Patient Population / Client Focus\n

You will work with a diverse clientele, including youth and adults, facing a variety of challenges ranging from anxiety and depression to more complex mental health disorders. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted Professional Counselor (LPC) licensure in the state of New Jersey.

• Minimum of two years of counseling experience, preferably in a clinical setting.
