ESSENTIAL CARE COORDINATION RESPONSIBILITIES

DUTIES RESPONSIBILITIES

The employee’s primary responsibilities include, but are not limited to:

Complete participant intakes, needs assessments, and person-centered service planning within

the scope of the program.

Coordinate referrals, appointments, transportation, benefits, health and behavioral-health services,

recovery supports, and community resources, as assigned.

Maintain regular participant contact and follow-up to monitor progress, address barriers, and

support continuity of care.

Collaborate with internal staff, treatment providers, community partners, and other authorized

supports to coordinate services and transitions.

Participate in care-team meetings and communicate significant updates, risks, or unmet needs

through established supervisory and program channels.

Respond to urgent participant needs in accordance with organizational procedures and promptly

escalate safety or clinical concerns to the appropriate supervisor or provider.

Maintain timely, accurate, and confidential notes, service plans, referrals, follow-up activity,

outcomes, and required reports.

Culinary Responsibilities

Plan, prepare, and serve meals and snacks in accordance with the program schedule and

approved menus.

Prepare nutritious meals that accommodate documented dietary restrictions and nutritional

guidelines when applicable.

Follow safe food-handling, storage, labeling, temperature-control, and sanitation procedures.

Maintain a clean, organized, and sanitary kitchen, food-storage areas, utensils, and equipment.

Monitor food inventory, minimize waste, and communicate grocery, supply, and equipment needs

in a timely manner.

Maintain kitchen equipment appropriately and promptly report maintenance or safety concerns.

Work collaboratively with program staff to coordinate meal service with participant schedules and

daily residential or program operations.

Maintain professional, respectful interactions with participants, residents, coworkers, vendors, and

community partners.

MINIMUM QUALIFICATIONS

Relevant care-coordination, case-management, human-services, community-services, recovery-

support, residential-program, or related experience.

Practical food-service, meal-preparation, kitchen, hospitality, or culinary experience sufficient to

perform the assigned meal-service duties safely and consistently.

Strong participant engagement, communication, documentation, organization, problem-solving, time-

management, and follow-through.

Ability to coordinate multiple participant needs while maintaining dependable meal preparation and

kitchen routines within a 30-hour weekly schedule.

Knowledge of trauma-informed, recovery-oriented, strengths-based, and participant-centered

practices preferred.

Ability to obtain and maintain ServSafe Manager certification within the first 90 days of employment, if

not already certified.

Ability to pass a Level II background screening and complete all required onboarding and training.
