Key Responsibilities

• Conduct comprehensive mental health assessments to understand each patient's personal history, emotional status, and specific needs within the hospice care setting.
• Develop individualized treatment plans tailored to the unique challenges faced by patients and their families, with a focus on mental health improvement and emotional support during end-of-life transitions.
• Administer evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and grief counseling, designed to help patients manage anxiety, depression, or any emotional distress linked to their illness.
• Facilitate both individual and family therapy sessions, creating a safe space for expression and coping strategies that promote emotional resilience.
• Collaborate closely with an interdisciplinary team of healthcare providers including nurses, physicians, and counselors to create a holistic and effective plan of care.
• Provide crisis intervention and support during acute mental health emergencies, employing de-escalation techniques and ensuring appropriate referral when necessary.
• Deliver psychoeducation to patients and their families about mental health resources, treatment options, and coping mechanisms, enhancing their understanding and management of emotional challenges.
• Maintain thorough documentation of assessments, treatment plans, and progress notes in compliance with regulatory standards and ethical guidelines.
• Participate in regular multidisciplinary team meetings and case reviews to engage in continuous professional development and enhance collaborative efforts.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• A current and valid Licensed Clinical Social Worker (LCSW) license in Virginia.

• Broad understanding of mental health disorders, treatment modalities, and trauma-informed care principles.

Compensation and Benefits
