Aspire Physical Recovery Center at Hoover is seeking an experienced Case Manager to coordinate patient care, discharge planning, and successful transitions following rehabilitation.

This is an excellent opportunity for someone with experience in hospital case management, nursing, therapy, rehabilitation, discharge planning, care coordination, or post-acute healthcare who understands what it takes to move patients safely and successfully through the continuum of care.

Our Case Manager serves as an important connection between patients, families, the interdisciplinary care team, community providers, and the patient's next level of care.

WHAT YOU'LL DO

• Coordinate discharge planning from admission through transition

• Meet with patients and families to identify discharge needs, concerns, and goals

• Develop and coordinate safe, appropriate discharge plans

• Arrange services, equipment, resources, and follow-up needs for discharge

• Work closely with nursing, therapy, physicians, and other interdisciplinary team members

• Communicate with families, responsible parties, hospitals, community providers, and outside agencies

• Identify barriers that could delay or complicate a successful discharge and help develop solutions

• Participate in care conferences and interdisciplinary meetings

• Coordinate transitions to home, assisted living, long-term care, or other appropriate settings

• Maintain accurate and timely documentation

• Help patients and families understand available resources and next steps

• Promote continuity of care throughout the transition process

WHAT WE'RE LOOKING FOR

• Previous healthcare case management, care coordination, discharge planning, rehabilitation, nursing, therapy, or related healthcare experience

• Strong understanding of the patient transition and discharge-planning process

• Experience working with patients, families, and interdisciplinary healthcare teams

• Excellent communication and interpersonal skills

• Strong organization, problem-solving, and follow-through

• Ability to manage multiple patients, priorities, and discharge plans simultaneously

• Comfortable navigating complex patient and family situations

• Strong documentation and computer skills

• Ability to build productive relationships with hospitals, providers, community resources, and post-acute partners
