Wellness CoordinatorThe primary purpose of the Wellness Coordinator is to direct the day to day medical and medication related services provided to residents in accordance with current federal, state and local regulations and following SHO policies and procedures.Essential Functions:Participate in the quality management program of the facility as directed by the resident services coordinator or administratorProvide guidance to QMAPs and/or PCPs regarding medication related issuesOrients and trains new QMAP and PCP staff on medication room procedures and documentationMonitor staff performance in medication room procedures and documentation; notify Resident Services Director of concerns related to staff performance in medication room procedures and documentationOrder/obtain medications and PCP supplies as needed to fulfill physician's ordersPrepare medication administration records and other medical file contents for new residentsDiscuss medication room procedures with new residents, including appointment management, medication procurement and physician's ordersUpdate all resident medication administration records as needed for physician order changesUpdate resident information record (face sheets) and medical chart information as neededComplete documentation and medication processing for resident transfers and dischargesDocument in communication log, resident chronological notes, flow sheets, etc. in an informative and descriptive manner according to SHO policyEvaluate content of record to assure complete documentation by QMAPs and PCPsAssess residents for changes in condition and implement proper interventionsMay notify family members, and/or case manager of change of conditionCommunicate with physicians, diagnostic services, hospitals and other outside providers as neededFill medication reminder boxes according to policiesAssist residents to schedule medical appointments and transportation as neededPrepare paperwork to send to physician appointmentsMay work as a QMAP or PCP to cover shifts as neededObtains a physician update for each resident on a semiannual basis.Respect resident privacy and maintain confidentiality of resident informationObserve and follow all safety protocols and fire and emergency plans; complete accident/incident reports as neededAlways respect resident rights, and report concerns to a supervisorOther duties as assignedWork Environment:Sitting, standing and walking throughout the day with intermittent drivingUsing office equipment such as computer, mouse, keyboard, printerMust be able to lift up to 30 lbs. comfortablyComfortable using and climbing on step stoolsCertain activities may require repetitive movements including climbing, crawling, crouching, pulling, pushingMay be subject to exposure to infectious waste, diseases, conditions including HIV and Hepatitis B viruses.Requirements:Competencies:Must have patience, and enthusiasm as well as the willingness and ability to handle difficult residents.Ability to communicate clearly both verbally and in writing, including the ability to provide non-technical assistance in a simple, straightforward mannerDemonstrate sensitivity to the concerns of residents and a commitment to dealing fairly with allFriendly, professional demeanor, positive disposition, tactful, patience and high ethical character even while dealing with difficult or upset residentsStrong organizational and interpersonal skills; attention to detailIntermediate computer knowledge and typing skills; working knowledge of Microsoft Office SuiteRespect for privacy and ability to maintain confidentiality of resident care informationEnjoys community involvement, resident interaction, and public representation of the organizationWillingness to be a team player and provide support to all departments as needed with the intent to provide the best care to our residentsBilingual a plusEducation and Experience:High school diploma or equivalent requiredPrior experience working in healthcare environment or working with an elderly population preferredTraining and designation as Qualified Medication Administration Person (QMAP) through program approved by the State of Colorado Department of Health requiredRead, write, speak and understand the English languageDemonstrated working knowledge of Microsoft Office Suite including Word, Excel, OutlookExpected Hours of Work of a Wellness Coordinator:This role may be full-time or part-time. Schedule is generally Monday through Friday. May be called during off hours to respond to emergency or unusual situations.
