Home Health Agency in District of Columbia looking for Pediatric Speech Language Pathologist Contractors for varied cases. Must have DC License. Must be a licensed SLP with 1+ year of experience Job Description We are seeking a compassionate Licensed Speech‑Language Pathologist (SLP) to provide high‑quality, in‑home speech therapy services to pediatric patients. This role offers the opportunity to work independently, choose your cases, and collaborate closely with a supportive, multidisciplinary care team. The SLP will evaluate, organize, and conduct medically prescribed speech language therapy for pediatric patients within the home health setting. They will direct the patient and caregiver in selected tasks to restore, reinforce, and/or enhance quality of life. Essential Duties and Responsibilities Performs accurate and comprehensive assessments. Completes OASIS data sets for "therapy only" cases, when required. Obtains physician orders for certification and recertification of home speech language therapy services. Ensures that physician orders are communicated to other interdisciplinary team members and Agency staff in a timely, accurate, and thorough manner. Ensures that changes in patients’ clinical status are communicated to other interdisciplinary team members and Agency office staff verbally and in writing in a timely manner. Coordinates care planning and delivery with the physician, patient and family/caregivers, other interdisciplinary team members and Agency staff to facilitate optimal patient outcomes. Educates patients, their families/caregivers, and other staff as appropriate, in appropriate speech language therapy modalities. Delivers speech language therapy modalities appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for patient’s age and physician orders. Provides, modifies or discontinues speech language therapy modalities based upon an ongoing assessment/reassessment of the patient’s clinical status and in accordance with physician orders. Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs, when appropriate. Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age. Appropriately assess and reassess a patient’s pain. Educate the patient and/or caregiver in effective pain management modalities. Provides speech language therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Identifies risk of injuries or threats to life and health in the patient’s environment, i.e., environmental/safety/mobility risks and implements appropriate interventions to reduce those risks. Addresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of speech language therapy. Performs transfer and discharge planning functions in accordance with Agency policy and applicable laws, regulations and standards. Completes and submits necessary documentation in accordance with Agency policy and applicable regulations. Prepares clinical/progress notes within 24 hours of the visit. Adheres to practice standards, including applicable federal, state, and local regulations and standards. Practices in a manner consistent with professional and Agency code of ethics. Maintains currency of professional knowledge by participating in continuing education offerings. Maintains patient confidentiality at all times. Demonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated time. Attends in service meetings, combined therapy and nursing service meetings, and Case Conferences. Supervises the PCA, if indicated. Communicates effectively with all those providing care. Confirms, on a weekly basis, the scheduling of visits with the Clinical Manager to coordinate necessary visits with other personnel. Identify and participate in Quality Assurance and Performance Improvement (QAPI) programs. Comply with all company policies, practices, and procedures. Other duties as assigned. Minimum Qualifications Education Master’s Degree in Speech-Language Pathology. Graduate of accredited SLP program. Licensure Current state-issued, District of Columbia, professional licensure. Experience At least 1 year as a Speech Language Pathologist. Certification CPR Certified. Successful completion of the PRAXIS Examination in Speech-Language Pathology. Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Transport: A valid driver’s license, auto insurance, and reliable car OR other reliable transportation are required. Completion of Criminal Background Check Other Qualifications and Skills Demonstrate excellent interpersonal and communication skills, interacting with patients and families in an empathetic and professional manner. Proficient in Microsoft Office Suite, record keeping, and routine database activities. OASIS and EMR experience Demonstrate strong critical thinking skills. Demonstrate attention to detail and strong organizational skills. Ability to interpret instructions and solve practical problems involving various concrete variables. Preferred Qualifications 2+ years of experience as a Speech Language Pathologist. Pediatric experience. Specialty Certifications from American Board of Child Language and Language Disorders OR American Board of Fluency and Fluency Disorders OR American Board of Swallowing and Swallowing Disorders. Physical Demands/Working Environment Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Spends one-third to two-thirds of the time standing, walking, reaching, sitting, kneeling, and crawling. Over two-thirds of the time is spent using hands and fingers, as well as handling, feeling, talking, and hearing. Work occurs in diverse indoor residential or childcare settings. Moderate physical activity is required, including strenuous therapeutic tasks. Must be able to lift at least twenty-five pounds. This position requires close vision abilities. The work environment includes moderate noise, such as that found in a home or business office with computers, phones, printers, and light traffic.
