Key Responsibilities\n

• Conduct thorough assessments for patients to identify and tailor mental health interventions that meet their unique needs.

• Act as a vital communication link among various stakeholders – including supervisors, family members, and other healthcare personnel – to maintain an accurate overview of each client’s progress.

• Establish and provide a tentative schedule for appointments, home visits, and evaluations, ensuring the patient's care plan is consistently followed and effective.

• Coordinate home visits and monitor adherence to treatment plans, documenting observations through chart reviews and interviews.

• Implement, update, and monitor personalized treatment plans in collaboration with an interdisciplinary team, ensuring they are tailored to patient feedback and progress.

• Participate actively in team meetings to discuss patient development, assess program effectiveness, and contribute to best practice strategies.

• Complete detailed monthly and quarterly documentation of progress notes, ensuring compliance with organizational requirements.

• Organize training sessions for staff to enhance competencies in program implementation, adherence to best practices, and documentation protocols.

• Provide ongoing support through follow-up interviews and home visits, ensuring holistic support for each client’s evolving needs.

• Maintain comprehensive records of client interactions and developments to facilitate informed care for all stakeholders.

• Act as a public liaison, strengthening connections with the mental health community to advocate for program development and resource access.
