What You’ll Do As a Home Health RN, you will: Perform comprehensive skilled nursing assessments Complete Start of Care, Resumption of Care, Recertification, and Discharge visits as assigned Complete OASIS assessments as required Develop and update individualized plans of care Provide wound care and other skilled nursing treatments Perform medication reconciliation and provide medication education Provide disease-management education Monitor patients for changes in condition Communicate with physicians and obtain necessary orders Help identify concerns early and reduce avoidable hospitalizations Educate patients, families, and caregivers Coordinate care with the interdisciplinary home health team Complete accurate and timely clinical documentation using WellSky What We’re Looking For Active Maryland Registered Nurse License Required Graduate of an accredited nursing program Reliable transportation Strong clinical assessment and critical-thinking skills Excellent communication and documentation abilities Ability to work independently in patients’ homes Home Health and OASIS Experience Preferred, but Not Required WellSky Experience is a Plus One Patient.
