Key Responsibilities\n

• Conduct comprehensive assessments of patients through in-depth interviews, case history evaluations, and observational techniques to gather essential data regarding wellbeing and mental health needs.

• Analyze collected data to identify core issues affecting patients, developing tailored therapeutic approaches or referrals to specialized services as necessary.

• Deliver high-quality counseling, therapy, and psychotherapy across various modalities, preparing individualized treatment and discharge plans that reflect each patient's unique circumstances.

• Maintain an active caseload, providing ongoing individual and collateral therapy, ensuring continuity of care and adherence to treatment plans.

• Regularly review client case files for accuracy, compliance, and documentation completeness, ensuring all required assessments, care plans, and progress notes meet clinical standards.

Patient Population / Client Focus\n

Your expertise will primarily support patients within the hospice environment, many of whom face complex emotional and psychological challenges associated with terminal illness and end-of-life transitions. You will also work closely with families to address their needs, providing invaluable support bereavement and other anticipatory grief processes.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution, reflecting a commitment to professional standards in social work practice.

• Active and unrestricted Clinical Independent Social Worker License (LICSW), demonstrating your qualifications to practice independently.

• At least 2 years of relevant clinical experience in a healthcare or mental health setting, with a strong preference for experience in hospice or palliative care.

Compensation and Benefits\n
