Licensed Practical Nurse — Long Term Care (Travel)

Overview
Licensed Practical Nurse serving a long-term care population in a travel assignment. Location: Newark, New York. Start date: 09/29/2026 for a weeks-long assignment on a 5x8 evenings shift. Weekly pay: $1,360-$1,462 per week. This 42-bed skilled nursing unit uses EPIC for charting, with a 16-hour orientation provided. First-time travelers accepted. Block scheduling with occasional floating per facility need. Orientation included and pre-approved time off of 1 shift.

Responsibilities
- Provide direct, compassionate nursing care to adults and geriatric residents on a 42-bed long-term care unit
- Perform wound care and pressure ulcer prevention, including wound vac management as needed
- Assist with ADLs, catheter and tube care (NG/G-tube, feeding tube management), and Foley care/insertions
- Administer medications (IM/SQ) and support phlebotomy and specimen collection
- Monitor respiratory status, including oxygen therapy and related devices; manage oral suctioning as required
- Support neuro/cVA care with stroke scales and post-stroke management; address behavioral health needs and implement dementia care and de-escalation techniques
- Collaborate with interdisciplinary team (PT, radiology, social services, pharmacy, unit secretary) to coordinate care and documentation
- Ensure accurate EPIC charting and timely documentation of assessments, plan of care, and flowsheets
- Maintain patient safety, comfort, and education for patients and families; participate in rounding and care conferences as scheduled

Requirements
- License: NY Licensed Practical Nurse with license in hand to submit
- Certifications: BLS required
- Experience: Minimum 1 year of clinical experience
- Charting: EPIC experience required
- Education: Not specified beyond LPN licensure; relevant practical nursing training
- Setting: Ability to work in a long-term care / skilled nursing environment
- Scheduling constraints: Block scheduling; weekends equal coverage per unit ratios
- Ratios: Days 1:21 | Nights 1:42 | Weekends 1:21
- Orientation: 16 hours
- RTO policy: No more than 10 days requested off
- Location policy: Candidates must reside more than 75 miles from the facility
- Pre-approved time off: 1 shift per posting
- Background checks: Reviewed case-by-case
- Pre-start modules: Completion required; up to 5 hours billable (may be counted toward onboarding)
